We’re committed to providing the safest possible surgical experience for your pet. Pre-surgical lab testing helps us detect hidden conditions that could increase anesthesia risks, allowing us to tailor care and ensure the best outcome. Please read this form carefully before signing.

Consent for Pre-Surgical Laboratory Examination

Your pet is scheduled for an elective dental or surgical procedure requiring anesthesia. Even under the best conditions, anesthesia carries some risk, and certain medical issues—such as early-stage liver or kidney disease—can increase that risk. These conditions may not show outward signs but can often be detected through blood testing before surgery.

While not required, we strongly recommend pre-surgical blood testing to check for abnormal chemistries and cell counts. Results can affect the type and amount of anesthesia used and provide valuable insight into your pet’s ability to undergo the procedure safely. Results remain in your pet’s record for future reference.

We prefer to collect and analyze your pet’s blood 2–3 days before surgery or dentistry. This allows time for necessary treatments before the procedure. If testing is done the same day, results may be available before anesthesia induction, but pre-testing is strongly encouraged.

Pets less than 6 years of age

  • CBC – Complete Blood Count: Anemia, Infection, Clotting Ability
  • BUN, CREA – Kidney Function
  • TP – Total Blood Protein
  • ALT, ALKP – Liver Function
  • GLU – Blood Sugar

Pets 6 years of age or older (Full Diagnostic Profile)

  • CBC – Complete Blood Count: Anemia, Infection, Clotting Ability
  • BUN, CREA, PHOS – Kidney Function
  • TP, ALB, GLOB – Blood Protein
  • ALT, ALKP, TBIL – Liver Function
  • GLU – Blood Sugar
  • AMYL – Pancreatic Function
  • CA – Blood Calcium
  • Electrolytes – Sodium, Potassium
